At Healthwatch Bucks we think it’s important that everyone in the county knows about the changes that are taking place in health and social care. We want to you to be kept informed and to have your say about the plans and how they are working out for you.
In our last article, we talked about the new community hubs in Marlow and Thame, which are early examples of the way services could be delivered across the whole county. If you have used either of these new hubs we would like to hear from you since similar hubs are planned for other communities across Bucks.
Those in charge of providing health and social care in our area have also started joined-up working in other ways.
Since November last year, our GPs, care providers, hospitals and drugs companies have been working to improve and simplify diabetes care. Nearly all of our GP surgeries now deliver the NHS Diabetes Prevention Programme.
Buckinghamshire Healthcare, which runs the Stoke Mandeville and Wycombe Hospitals, is working with partners across the county to join up services for those suffering from conditions, which affect the joints, bones and muscles. The new service is being rolled out now and is expected to be fully live from April 2019.
GPs in care homes in Aylesbury are working with experts to introduce remote video consultation between healthcare professionals and patients to address rising numbers of emergency hospital admissions from care homes and ease demands on GP time.
There has been a significant and sustained reduction in emergency admissions since the care homes have increased their use of this service.
Many other projects are planned for the next 12 months, including more community hubs and making it easier to get GP appointments at evenings and weekends. We are also promised better access to mental services and improved prevention, diagnosis and treatment of cancer.
All of us who live and work in the county have an important part to play in shaping the projects to create a joined-up health and care system here.
