We’ve had a great response to the discharge survey so we’ve extended the closing date. You can now submit your comments about hospital discharge until December 25th 2016. Remember, the survey doesn’t cover visits to A&E. At Healthwatch Bucks we’re most interested in discharge from Wexham Park but you can use the survey for any hospital.

There is some additional good news from Healthwatch WAM. They have secured permission to survey patients waiting in the Discharge Lounge at Wexham Park. This a great opportunity for people to give their feedback while their experience is fresh in their mind. Some of our volunteers will be joining those from the other Thames Valley Healthwatch also involved in the survey.

    We must stop using the term ‘discharge’.
    We should all, staff, hospitals, & patients AND NHS England and all, be using the term ‘Transfer of care’.

    The NHS need to create the impression for patients that their care is continuing but in another location or with another team. In may cases patients describe the feeling experienced when they move from hospital care back to the community as ‘falling off a cliff’. This is totally unsatisfactory. It is not a new problem and is found across the country. Time to act not talk.

    For many years the transfer of care has been poor. Poor communications, poor provision of social support, poor planning with relatives & carers, poor transport arrangements, lack of assessment for therapy services & social care support.
