Mental Health Discharge Summary headings are currently being updated by the Professional Record Standards Body (PRSB) and the Royal College of Psychiatry.
We need your views on the headings and their descriptions which will structure the updated electronic Adult Mental Health Discharge Summary documentation. This is the document that will be sent to the person’s GP following their discharge from a Mental Health Care hospital or acute care facility back to the care of their GP. We want to standardise those headings that should be included in all Adult Mental Health Discharge Summaries so everyone is completing the same type of information and in the same format for every person.
It is very important that the discharge summary to GPs and other people involved in caring for the client in the community, including the patient themselves and their carer(s), records all the information needed but nothing extra.
In the electronic discharge summary it is envisaged that information from the person’s computerised medical record, such as personal details, laboratory test results, medications and whatever else is held on the hospital’s computers, will be automatically fed into the discharge summary. In the hospital environment the care professional completing the discharge summary will only complete the information that is relevant and applies to each client (the headings that do not apply to that client will be ignored).
