We recently posted a news story about the “failure” of Southern Health Foundation Trust (SHFT) to investigate a number of unexpected service user deaths. As stated in our post, the reports appearing in the media were based on a draft obtained by the BBC. The full report, commissioned by NHS England, has now been published. A number of organisations have published responses to the full report including Southern Health Trust. NHS England published a detailed response (link below) to the national recommendations.  Locally, Chiltern Clinical Commisioning Group (CCCG), which commissions some services from SHT, gave this response.
